Teacher Certificate Renewal

Conference Services is the sole record keeper for CPDUs issued by Illinois State University.  If an instructor chooses to become a preferred provider and provides CPDUs (and the required record keeping), the workshop cannot be advertised as an Illinois State University workshop.

• Flow Chart - shows how the process works on campus.
• Notice of Professional Development - to be sent to Conference Services at least 7 days before your event. 
• Cover Sheet for Proposals - this form must be signed by department chair to show workshop has been approved. This should also be sent to Conference Services at least 7 days before the event.
• Workshop Roster - to use for workshop sign in
• Evaluation and Evidence of Completion for Workshop, Conference, Seminar, etc. - Evaluation - must be handed to workshop participants.
• School District Proposal for Illinois State University Professional Development Activities for Teacher Recerticifation (Hosting a Local Workshop)

Evidence of Completion

Conference Services will send this form to participants after your event.  This is the form teachers need to prove their attendance.
